Orchid disintegrating from inside out

Has anyone seen something like this? The stem just hollows from the inside, and the plant flops over and dies. This is the second time this has happened a couple weeks after the order. Both of them are from a highly reputable vendor. I've never seen anything like this before in any of the other plants I've owned. I'm a bit wary of even asking for a refund after having asked for one just a few weeks ago.
