Why do some growths have two leaves, others only one leaf?

I'm wondering why on plants where I've noticed this such as Laelias and Cattleyas, there will be growths that only grow one leaf whereas other growths produce two leaves on the same plant?

What I've learned is just about the only thing you can usually try is coating the infected area with cinnamon and sphag-n-bag it. I've never had any luck with Phals though when crown rot sets in....
