Question White mushroom in Phalenopsis mix

Hi!
I'm new to orchids, I only have this one Phalenopsis. I'm not sure what kind, it was a gift. I remember it's a "just add ice" orchid and it flowers pink with white stripes.
I haven't changed the potting mix since I got it. It hasn't moved locations either. I used a little bit of liquid fertilizer on it a few months ago when it wasn't blooming. I made sure to use a fairly low rate, at least for regular plants.
Anyway I noticed this little mushroom this week. There's just the one. It's about 5mm across the cap. Is it a problem? Do I repot the orchid with fresh mix?

Definitely needs repotting... mushroom is a sign of excessive moisture and broken down mix. Remove it from the pot, clean off as much of the old mix as you can. If the outer part of the roots is brown and mushy, remove it. You can leave any of the stringy centers of roots that don't break lose easily - that are capable of hydrating the plant (inefficiently but better than nothing) and will serve to stabilize the plant in the new (relatively large) bark that it should go into. Water it well, then let it dry out for a few days before watering again. (New bark will dry out much faster than the "mud" that it no doubt is sitting in. New bark will encourage it to produce new roots, which I am sure that it needs.
