What is going on with this phal? Pics are of the top, bottom, and light through the leaf.

hmm how do it roots look? How much light do you give it? and what are your temperatures?

Nice green roots. It gets filtered light from a window - not too much. We keep the house at about 68-70.

was the leaf touching the glass at any stage? Looks like a mild case of too much sun and not enough humidity around the plant try bringing plant back from the light source or the use of a sheer curtain

No it hasn't touched the glass. They sit off to the side of the window on a table.

I don't see anything except a few "bubbled" areas (which I tried to take a picture on in #2). As a precaution I gave it a soapy bath (dawn and alcohol) when I got it (I've done this to all of my orchids).

I did notice that 2 of its buds are turning brown (it has 1 huge spike with 3 additional spikes off of it). I don't know if it is related or not.

I thought it maybe was scale insects or aphids but if there is nothing under the leaves then I dont think it would be that.

Is there any way a slug or snail could have got on your orchid?

it's not the result of insects or dieseases i have a phal that its the same like i mention before in my post the leaf has over heated either by direct sun or heating, while not having enough humidity and moisture to compensate and it has distored the cells within the leaves there isn't much you can do about it , hope the phal will be strong and rebuild new leaves.
In the first picture the leaf is very yellow-green with horizontal streaking that mean the phal is at its limit on the amount of light it can take. Don't forget your winter sun is increasing its strength everyday
