Hi Jasen, what's your secret.....other than living in Florida!  
I've had a Phaius tankervilleae for years and it's never done a thing. It puts out a few new leaves but just as many die off so it doesn't really get any bigger. Of course it's never flowered.
What culture do they like, light, water, fertilizing etc?
It has a tiny growth on the side of the bulb at the moment, I don't suppose it could be a spike? Where do the spikes generate from on the plant?

No secret really....medium light, feed once a week, and it gets daily misting from my greenhouse misters. I'm sure the mistings aren't the cause. In my experience with orchids, when a plant won't bloom, 9 out of 10 times it is due to improper light. Whenever I get an orchid that won't bloom....I move it to a place with more light, and it blooms. If you get the light right, the plant will almost care for itself.
When it does bloom, it will emerge from the base of the mature p-bulbs. Mine bloomed about two months ago, so it may still be close enough to bloom time for you to get a spike. Hopefully, the new nubs will be spikes. Sometimes you can give them a little boost by feeding them 30-10-10, or a high nitrogen fertilizer. Then, within 2-3 days (no longer) feed them again with a blossom booster (10-30-20), or something with a similar N-P-K number. Good luck!

Phaius tankervilleae
Both my Phaius' are kept outdoors in good weather which means from April till November +-. They get lots of light including some direct sunlight according to the time of day and season. My tankervilleae is growing lots of new leads. It had 5 when I bought it so I expect at least 5 new spikes when it gets around to that. I bought it in the spring when it was in spike and not blooming yet. My other Phaius (akk, I don't have my list at hand) is smaller but it is coming along well, too. I have wound up fertilizing all my plants semi-weekly, semi-weakly (just lazy)
Give it more light. If mine can take the bright light I give them in my climate, surely yours can take more, too. Just don't stick them into the sun all of a sudden. Acclimatize them gradually.

So the nubbies may be new growth ...
I think these like to be pretty moist ...
