Default Psychopsis Roots-- Died before hitting medium?

Hi guys!
I have a big beautiful Psychopsis Mendenhall 'Hildos' and I've had a question in my head that's been bothering me for a while so I figured I'd go ahead and get if off my chest. A couple months after I got it, I noticed it started three new roots from the base of one of its newer growths, and they had great green tips. But about a week later I saw the tips had browned and withered before they even reached the medium. Would this have been a watering issue on my part? The rest of the plant is fine and it doesn't seem to have noticed the loss of those little roots, but it kind of bothered me to see those die off so quickly.

It's hard to get a fair idea about how or why the roots died unless there's a fair amount of detail about the situation - eg. where the orchid was placed, lighting and humidity conditions, frequency and amount of watering, whether any fertilisers/chemicals were used, potting media, temperature etc.

The very nice thing here is - you mentioned that the rest of the plant looks fine - including the rest of the roots.

Oh whoops, yeah that would be helpful!

It grows in my bedroom on a shelf that receives southern exposure, but the window is shaded in summer to prevent burns. 50-70% humidity. I water once the medium has just dried out and I water all my orchids from below. I fertilize once a month during winter with liquid fertilizer that is diluted to a quarter strength instead of the half strength that the bottle recommends. My temps are anywhere from 65-70 in the winter and 70-78 in summer. It has a lot of growths and about 4 or 5 pbulbs. Its in the potting medium it came with so I'm assuming bark and perlite and it has moss on top

Roots die back mostly because they aren't happy with the humidity, or because they were mechanically damaged. This can mean bumped or nibbled.

I have been spraying the new roots of my Psychopsis with a water bottle filled with distilled water that I use for my air plants. I am not sure how that would work with a medium of bark or moss as I use red lava rock as a medium and it dries very quickly.

I read somewhere they really prefer close to 80% humidity so that would probably be a safe bet on the issue here
