Unhappy Rootless phal on a heating pad lost its roots - need your help.

Hi.

So a while ago I've posted a thread about my almost rootless phalenopsis in a bag. It had two small roots, and after a discussion on this forum, I've planted it in a pot on a heating pad, covered with a bag.
I was watering it every 4-5 days as I was told, and it seemed happy for some time.
Unfortunately today I've noticed that it was slightly tilted, and after investigation it came to me, that these roots have died. Apart from that it looks quite healthy.
There are few more fresh roots growing out of it, but I'm really concerned what would cause the previous ones to die. It wasnt sogging wet or anything like that, also from time to time I took the bag off to let it breathe, and used a 3:1:3 fertiliser every 2 weeks.
Dunno if this might be the issue, but some time ago it had a bit of fungi on the leaf (a dying leaf), so I cutted it off with a sterilised blade.

Today I have taken it out of the pot, cleaned the potting medium a bit, so there were no dead roots whatsoever, and after that I've potted it back. Im really worried that I might be doing something horribly wrong, so I need your help.
I can post some pictures if you want.
Also is this normal for phals to have two roots growing from a single hole?

The roots adapt to the medium they grow in. So losing the old roots isn't a big deal if you're getting new roots now. The new roots are adapting to the way it's now being grown. So depending on the length of the new roots, time to figure out what medium you want to use and get it started transitioning.

Pictures would help. I don't understand the two roots coming out of same hole part...
